1. I've read the "Members' Standards" and believe that I qualify for membership with MTBG. 2. I swear to protect the confidentiality of fellow members' statements/conversations as well as any identifying factors regarding their clients. 3. I swear to abide by MTBG's code of ethics. 4. I understand that after filling out the application I must sign and date it. Also, within 30 days of finishing this application, I must supply copies of my credentials, photo ID, and proof of insurance. 5. I will visit the "Member Payments" page to pay my fees or included it with my application and above listed document copies. (Make check or money order out to either "MTBG" or "Massage Therapist and Bodyworkers Guild". ) |
